- Fill out the "CONTACT PERSON INFORMATION"
- Place a check mark in the box next to the course you wouldlike your staff to attend. Up to six (6) trainees per shelter maybe registered.
- Print the name and title for each of the registrants.
- Fax or e-mail the completed registration form as soon as possible. Please note thatspace is limited and classes are filled on a first come firstserved basis.
